Locating Your Survey
	When it is time for you to complete an evaluation you will receive an email message providing you with instructions about how to access your survey. In most cases, you will be able to click on a hyperlink within the email message and connect to the correct survey. Since occasionally there are difficulties with email, we have provided you with a couple of options below.

Please select the option that is most appropriate for your situation

1. You received the email message but the hyperlink is inactive.

   Your email message includes a Survey # and an Participant/Attendee ID # for the class. Enter the numbers and your last name below (your name is an extra security measure and your Participant/Attendee ID will give you credit for completing the survey).

   Your Last Name Participant/Attendee ID # Survey ID #

2. You did not receive an email message and/or you have another type of question.

   Please email me at helpdesk@trainingevals.com with the following information.

   Your Name
   What survey you are trying to access
   Your Telephone Number
   Your Question or Problem
